Choose Skin Health Materials

ASDS sends complimentary supplies and marketing materials to help members' screenings be successful. These materials are intended to educate the free screening patients about sun-safe behaviors.

Supplies include:

• Patient screening forms to document any findings
• Neutrogena® sunscreen samples
• Patient education brochures (printed version includes $2 Neutrogena® coupon; Spanish version available digitally)
• Promotional posters
• Physician name and screening date / location listed on the "Find a Free Skin Cancer Screening" public database
• A Choose Skin Health volunteer certificate for display in the office and online (members must submit an attestation form and completed screening forms after the event to receive a participant certificate)
